Obtain your application form from the head offices of the respective Public Health Schools. The form fee is Rs. 200. - Prepare Your Documents
Make sure you have the following:- Matric Pass Certificate with date of birth
- F.Sc (Pre-Medical) Pass Certificate
- Marks Sheets (Matric and F.Sc)
- Domicile Certificate and PRC Form-C
- Character Certificate from your last school
- CNIC
- Four attested passport-sized photos
